Position Summary:

VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Hesperia, CA. Key Responsibilities:

• Provide physical therapy services in home health setting to patients in Apple Valley and surrounding areas
• Conduct patient visits with 1:1 ratios, up to 6 patients per day with 30 productivity points standard
• Utilize EPIC charting system and Oasis documentation for patient records
• Develop and implement home exercise programs for patients
• Provide patient and family education regarding treatment plans and care
• Treat various conditions including post-operative cases, neurological disorders, orthopedic conditions, and chronic pain
• Work with adult and geriatric patient populations
• Maintain current BLS certification and state licensure
• Provide reliable transportation for patient visits with occasional travel radius of 50+ miles
• Work 8-hour day shifts with self-scheduling responsibilities
• Complete required modules and training programs pre-start and annually
• Work one weekend shift per contract as required
• Collaborate with interdisciplinary team including other therapy services and social services
• Float between facilities within state as needed to meet facility requirements
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Complete non-billable training modules averaging 16 hours with self-reported time tracking
• Utilize facility-provided communication devices when required for patient care coordination

Job Requirements:

• Active, unencumbered license in CA
• Minimum of 1-7 years of relevant experience, depending on specialty
• Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
• Experience with EMR systems (Epic, Cerner, as applicable)

Other Required Documentation:

• Current Resume listing detailed work history (MM/YYYY format)
• Two current clinical references
• State-required professional license
• Physicial exam (completed within the past 12 months)
• TB skin test or Quantiferon Gold (within the past 12 months)
• Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
• Current Tdap and Flu vaccinations
• Must pass background check and drug screening
