Position Summary:

VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Hesperia, CA. Key Responsibilities:

• Provide physical therapy services in home health setting • Conduct home visits to patients in Apple Valley and surrounding areas • See up to 6 patients per day maintaining 30 productivity points standard • Complete OASIS documentation and assessments • Utilize EPIC charting system for patient documentation • Provide physical therapy services for adults and geriatric patients • Treat various conditions including post-surgical rehabilitation, neurological disorders, orthopedic conditions, and spinal injuries • Develop and implement home exercise programs for patients • Provide patient and family education on treatment plans • Maintain current BLS certification and state licensure • Work one weekend shift per contract • Complete required modules and training programs pre-start and annually • Self-report time spent on non-billable training modules • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty • Maintain reliable transportation for patient visits • Float between facilities within the state as needed • Use facility-provided communication devices when required • Adhere to facility policies and procedures for patient care delivery

Job Requirements:

Active, unencumbered license in CA

Minimum of 1-7 years of relevant experience, depending on specialty

Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements

Experience with EMR systems (Epic, Cerner, as applicable)

Other Required Documentation:

Current Resume listing detailed work history (MM/YYYY format)

Two current clinical references

State-required professional license

Physicial exam (completed within the past 12 months)

TB skin test or Quantiferon Gold (within the past 12 months)

Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)

Current Tdap and Flu vaccinations

Must pass background check and drug screening
