Position Summary:

VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Napa, CA. Key Responsibilities:

• Provide physical therapy services to home health patients in a 1:1 ratio • Conduct up to 6 patient visits per day meeting 30 productivity points standard • Maintain current BCLS certification throughout assignment • Work one weekend shift per contract period • Utilize EPIC charting system for patient documentation • Provide own reliable transportation for patient visits within service area • Complete required pre-start and annual training modules (average 16 hours, non-billable) • Self-report time spent on training modules with completion recorded in Workramp transcripts • Float between all facilities within the state as required • Work 8-hour day shifts without accommodation for 4x10s or 3x12s • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting • Maintain valid state physical therapy licensure • Utilize facility-provided communication devices when required for patient care • Apply clinical skills across various conditions including acute care, rehabilitation, spinal conditions, neurological disorders, and post-surgical care • Develop and implement home exercise programs for patients • Provide patient and family education on treatment plans and care management

Job Requirements:

Active, unencumbered license in CA

Minimum of 1-7 years of relevant experience, depending on specialty

Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements

Experience with EMR systems (Epic, Cerner, as applicable)

Other Required Documentation:

Current Resume listing detailed work history (MM/YYYY format)

Two current clinical references

State-required professional license

Physicial exam (completed within the past 12 months)

TB skin test or Quantiferon Gold (within the past 12 months)

Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)

Current Tdap and Flu vaccinations

Must pass background check and drug screening
