We’re looking for a  Service Support Specialist  to join our team!
At Mosaic, we empower people to live as independently as possible.  As a Service Support Specialist, you’ll be responsible for receiving, accepting and documenting all new referrals and developing and maintaining relationships within the community.  You will also coordinate the Direct Care Workers (DCW) from inception to assignment and beyond, ensuring services are provided in a manner that is consistent with our mission.

What you’ll do in this role:
Fosters  relationships with Managed Care Plans, Case Managers, community partners, and referring entities for home and community based services (HCBS)
Assists with  intake assessments of new clients to review conditions, treatment and expectations for care
Assists in developing a service plan to address identified individual client needs in accordance with company procedures
Assists in conducting ongoing client home visits to ensure proper care and standards are met
Assists to ensure all contractual obligations are met by direct care workers, review and correct any deficiencies in a timely manner
Assists to ensure notification to direct care worker if service termination occurs
Adjust and manage scheduling changes
Assists with the Investigation and research of incident and grievances
Support with DCW onboarding and client monitoring procedures
Coordinate with service delivery to update revisions

You should have:
High school diploma or equivalent required.
Two years of supervisor experience required (may be substituted with college studies, equally).
Preferred minimum one year of experience with service coordination or delivery.
CPR/First Aid certification, Article 9 and Principles of Caregiver certifications
