Job Description
Sunshine Homecare Services is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to provide quality, patient-centered nursing care to medically fragile children with complex health care needs in their homes. As an LPN, you will work under the direction and supervision of a Registered Nurse (RN) to ensure that our clients receive safe, effective, and personalized care.
Position Details- Location: Westchester, Orange, Dutchess, Putnam, Ulster
- Employment Type: Full-Time / Part-Time/Per-Diem
- Schedule: Days/Nights/ Weekends
- Rate: $40.00 - $42.00
- Provide direct patient care to infants, children, and adolescents according to the active plan of care, as given by the physician and Supervising RN.
- Administer medications and treatments as prescribed by the physician
- Observe, record, and promptly report client condition changes to the supervising RN
- Collaborate with clients, families, and other healthcare providers to promote continuity of care
- Document timely clear, concise, and accurate clinical notes daily in accordance with the plan of care.
- Accompany pediatric clients to school to provide 1-on-1 care when assigned
- Use appropriate infection control procedures and safety measures
- Comply with all company policies, practices, and procedures.
- Be prepared to respond to medical emergencies and provide appropriate interventions as necessary.
- Notify the RN Case Manager of any changes in the patient's condition.
- Provide and document education to patients and their families on specific details of diagnosis, treatment, and prevention.
- Graduate of an accredited LPN program
- Valid NYS LPN License
- Minimum 2 years of home care and/or pediatric experience preferred
- Ability to work under the supervision of an RN while practicing within LPN scope of practice
- Valid Practical Nursing Degree/Diploma
- Active New York State LPN License
- Current CPR Certification
- Strong organizational, writing, and communication skills
- Basic knowledge of vents, trachs, oxygen, feeding tubes, urinary catheters, ostomies
- Understanding of pediatric disease processes and medication interactions
- Ability to work with clients of all ages and backgrounds
- Reliable transportation required
- Ability to lift over 20 lbs., move and carry medical equipment
