What You’ll Do

As a Nurse Practitioner at Alteas Health, you’ll be at the forefront of patient care, focusing on:

Diagnosing and managing acute and chronic health conditions.
Educating patients on effective self-management strategies.
Collaborating with senior care facility teams to ensure optimal patient outcomes.
Ordering and interpreting diagnostic tests to guide treatment plans.
Maintaining thorough and accurate patient records.

You’ll work closely with our Senior Clinical Operations Manager in a supportive and fast-paced environment that fosters innovation and teamwork.

What We’re Looking For

Qualifications: A valid Nurse Practitioner license in Illinois with at least 6 months of experience.
Transportation: Reliable, insured vehicle for travel between facilities.
Character: Compassionate, resilient, and committed to delivering exceptional care.

This exciting opportunity has a base salary of $120,000 to $145,000 annually, in addition up to a $5,000 sign on bonus and an estimated $6,000 - $10,000 annual compliance and productivity incentives!
