Responsibilities include diagnosing, treating, prescribing medications, ordering diagnostic tests, and educating patients on health management. The Advanced Practice Provider (APP) (Nurse Practitioner or Physician Assistant) works within an outpatient primary care clinic as part of a collaborative team, which operates under a comprehensive, coordinated care model to ensure high-quality patient outcomes.
