Family Practice/Primary Care Nurse Practitioner at Southeast Medical Group summary: The Family Practice/Primary Care Nurse Practitioner provides comprehensive outpatient care as part of a collaborative primary care team, managing acute and chronic conditions. Responsibilities include diagnosing, treating, prescribing medications, ordering diagnostic tests, and educating patients on health management. The role emphasizes high-quality, coordinated, evidence-based care to improve patient outcomes in a primary care setting. The Advanced Practice Provider (APP) (Nurse Practitioner or Physician Assistant) works within an outpatient primary care clinic as part of a collaborative team, which operates under a comprehensive, coordinated care model to ensure high-quality patient outcomes. Acting as an extension of the primary care physician, the APP delivers both chronic and acute care services to patients. ESSENTIAL JOB FUNCTIONS The Advanced Practice Provider (APP) delivers patient care in accordance with their licensure and certification. Diagnoses and manages both acute and chronic conditions, including diabetes, hypertension, kidney disease, and other chronic illnesses, as well as minor injuries and acute medical issues such as infections or exacerbations of chronic diseases. Orders and interprets diagnostic tests, including lab work, x-rays, and EKGs. Prescribes medications and other treatments as necessary to address identified conditions. Educates patients about their medical conditions, treatment plans, and expected outcomes to ensure they are well-informed and engaged in their care. Collaborates as part of a comprehensive, multidisciplinary team to provide high-quality, evidence-based care. Perform accurate and timely documentation of histories, subjective findings, physical exam, plans, prescriptions and follow up in EMR within 72 hours of the visit. Performs additional duties as assigned to support patient care and organizational goals. MINIMUM REQUIREMENTS Graduate of an accredited PA/NP Program Current, valid and unrestricted State Licensure Valid DEA Current CPR - Basic Life Support from the American Heart Association PREFERRED QUALIFICATIONS Three (3) years of experience working in a primary care clinic. ACLS / PALS are a plus. Bilingual in Spanish. TOTAL COMPENSATION PACKAGE Great earning potential which includes a base salary plus a lucrative bonus plan.
