Case Manager Overview The Case Manager is responsible for assessing client needs, developing comprehensive care plans, and ensuring access to essential services.
They play a key role in advocating for clients and monitoring their progress toward stability and success.   Responsibilities Client Assessments :
Evaluate individual needs and create personalized service plans.
Resource Coordination :
Connect clients with healthcare, housing, education, or employment assistance.
Monitoring & Follow-Up :
Track progress and adjust service plans as needed.
Crisis Intervention :
Provide support during emergencies or difficult situations.
Documentation & Compliance :
Maintain case records and ensure compliance with regulations.
Collaboration & Referral Management :
Work with social workers, healthcare providers, and agencies to support clients.   Qualifications Education : A bachelor's degree in social work, psychology, human services, or a related field is typically required.
A master's degree in social work (MSW) or counseling is preferred for higher-level roles. Experience :
2-5 years of experience in case management, social services, or counseling.
Technical Skills :
Familiarity with case management software, documentation tools, and client tracking systems.
Communication & Interpersonal Skills :
Strong ability to connect with clients and collaborate with service providers.
Problem-Solving & Advocacy :
Ability to assess client needs and advocate for necessary resources.
Cultural Competency :
