ROLE AND RESPONSIBILITIES
Leads and coordinates case management services, ensuring Implementation Strategies (IS's) are developed, reviewed, and implemented in compliance with agency and state regulations. Advocate for participants by coordinating necessary medical, educational, vocational, and social services. Maintain regular contact with participants and their families to support informed decision-making, address barriers to community integration and ensure their day-to-day needs are met. Respond to participant crises (on-call required) and provide direct support when necessary. Ensure accurate and timely completion of participant records, monthly progress notes, service agreements, and compliance documentation. Monitor service plan implementation through site visits and maintain adherence to regulatory and accreditation standards. Facilitate communication and provide guidance and support between participants, families, program staff, and external service providers. Lead team meetings (annual and special), assist with appointments and participate in problem-solving to enhance service delivery. May assist with mentoring new staff, provide coverage for absent staff, and performs other duties as assigned to maintain effective department operations.
PREFERRED SKILLSKnowledge of theories, methodologies, technologies, and Federal and State laws and regulations pertinent to the field of developmental disabilities. Operate personal vehicle, as necessary, for transportation needs of persons served and/or movement between program/worksites. Utilize and operate Agency computer equipment for word processing/data management, telephone systems and other common office equipment.
