Duration: 13-week contract
Position: Inpatient Pharmacist
Job Responsibilities:
Identifies and resolves actual or potential medication-related problems.
Takes ownership of pharmacy-related issues and follows them to completion.
Provide accurate drug information to staff, providers, and the community.
Assist physicians and residents in developing optimal drug therapies for each patient, including pharmacokinetic dosing and monitoring services.
Establish a pharmacy practice specialty or an area of expertise and act as a resource for this specialty, providing education, monitoring, and other support about this specialty to improve the quality of the department.
Documents and monitors all interventions and their outcomes.
Performs medication order entry consistent with current department policies and procedures.
Coordinates the pharmacy's daily distribution process.
Represents the pharmacy in hospital-wide committees. Participates in pharmacy committees.
Provides written and in- service education regularly.
Performs other projects or duties as required or assigned.
Qualifications:
The ideal candidate should have a CAlicense
PharmD degree or Bachelors degree in Pharmacy from an accredited program.
A certified Pharmacotherapy Specialist (BCPS) is desirable.
A Pharmacy residency is desirable.
