Job responsibilities:

Document evaluations, treatments, and outcomes in the medical record. Evaluate functional status and develop a treatment plan aligned with clinician goals and patient needs. Communicate functional changes and barriers to the care team to support care planning. Work 40 hours per week with 10-hour shifts.
