Job responsibilities:

Provide patient and family education related to the plan of care and discharge needs. Work 12 hours per week with 8-hour shifts. Assess patient status and respond to changes using established clinical protocols. Maintain infection prevention practices and safe medication administration standards. Support safe transitions of care, including handoffs and care plan updates.
