Job responsibilities:

Support safe transitions of care, including handoffs and care plan updates. Maintain infection prevention practices and safe medication administration standards. Perform focused reassessments and escalate concerns through appropriate clinical channels. Document assessments, interventions, and patient progress in the medical record. Work 36 hours per week with 12-hour shifts.
