Job responsibilities:
Educate patients on treatment plans, follow-up instructions, and self-management strategies.
Administer ordered therapies and monitor for adverse reactions or changes in condition.
Work 40 hours per week with 8-hour shifts.
Document care delivered, patient response, and communications in the medical record.
Perform medication reconciliation activities as directed and communicate discrepancies.
