Help patients perform specific exercises and use therapeutic equipment safely in the home environment. Educate patients and family members about exercises and activities to continue between visits. Adapt interventions as needed within the scope of the treatment plan and state laws. Ensure all documentation is accurate and completed in a timely manner according to agency and regulatory standards. Work Environment: The role is based in patients' homes, requiring travel within the community and adaptability to various home settings.
