Social Worker/Bereavement Coordinator- Part Time

Hiring a part time Social Worker/Bereavement Coordinator to work in the Community Health Department. This position is 16 hours in a two-week period.
Key Responsibilities
• Responsible for making face to face visits with patients or families in the hospice program
• Attend bi-weekly IDG meetings
• Complete hospice assessments related to the care plan
• Is responsible for the hospice bereavement program
• Participates in agency and community programs such as Hospice Tree of Lights and Living Through Loss
Qualifications:
Education : Must have one of the following: a Masters of Social Work degree from a school accredited by the Council on Social Work Education and one year of experience in a health care setting;
OR have a Bachelors degree in Social Work from an accredited institution by the Council on Social Work Education
OR a Bachelors degree in psychology, sociology, or other field related to social work with at least one year of experience
Position is 16 hours in a two-week period. Working hours are Monday through Friday, 8:00am to 4:30pm. Days may fluctuate based on patient need. Applicant must pass background check, drug screen and health assessment including applicable vaccinations prior to hire. Position open until filled.
