Purpose:
This is a key role in the success of the outpatient home dialysis department. This role is a dual role that requires clinical skills and knowledge as well as administrative duties, including running and managing the front desk operations while also assisting patients. This includes assisting in the daily functions of a home dialysis facility, working with and reporting to home-training nurses and nephrologists.
Responsible for all front desk activities, including greeting and checking in patients. Additionally, this role provides clinical support, including the setup and operation of home dialysis machines, assisting patients in obtaining weight and vital signs, monitoring patients, reinforcing education under RN supervision in compliance with regulations set forth by the corporation, state, and federal agencies.
This role is the "face" of the home dialysis unit. This position requires a high level of customer service as well as attention to detail and working in a fast-paced and multitasking environment. This role will greet the patient, check the patient into the system and then invite them into the clinical space and provide all screening and clinical rooming activities as well as close the visit and schedule follow-up care. This role also includes additional clinical responsibilities such as blood draws, lab prep and various quality and regulatory requirements.
Assist in the maintenance of a safe and clean working environment. Supports the commitment to the Quality Assessment Program Improvement (QAPI) and CQI Activities, including those related to patient satisfaction, and actively participates in process improvement activities that enhance the likelihood that patients will achieve the Quality Assessment Program Improvement (QAPI) goal.
Key Job Activities:
Register new patients in the system, while utilizing Sharesource and keeping accurate patient orders updated, and tracking patients in hospital.
Call to schedule lab and clinic appointments, answer and screen calls. The reminder calls for all scheduled patients. Obtain medical records as requested
Draw monthly labs, administer ESA/Vaccinations as ordered
Insurance verification on all patients, authorized medication refills, pre-authorizations for dialysis
Keep inventory of all supplies for Baxter, Sinai Lab, Medications/Vaccinations and check supplies monthly for expiration. Order new patient supplies along with equipment needing to be picked up through Baxter. Receive and store incoming supplies and medications appropriately
File daily lab reports/paper documents, retrieve incoming mail and process outgoing mail
Centrifuge/Prepare specimens for transportation along with arranging pick-up/transportation for lab specimens. Prepare and send out GOH specimens
Draw monthly Gift of Hope for transplant patients
Keep an updated log of patients on the transplant list
Stocking patient rooms along with preparing rooms for daily training/lab draws/sterile procedures
Ensuring tracking logs are completed for the following: refrigerator temp, eye wash station, fire extinguisher, emergency medication and glucometer logs, patient hospitalization tracking
Remove biohazard waste per policy
Quarterly patient emergency education review and sign off. Quarterly chart audits
Prepare materials for PD education consultations, prepare check requests, partner with AP for outstanding invoices
Send out inactive charts for storage
Education and Work Experience:
Medical assistant diploma required
Dialysis experience a plus
Knowledge and Skills:
Experience Preferred in EPIC, Infian and MEDITECH software systems
Certifications/Licenses:
CPR certification required
