Summary Under the supervision of the Refugee Health Promotion Program Supervisor, the Case Manager is responsible for ensuring that health and wellness services are provided in a planned, effective and timely manner to eligible clients. The Case Manager delivers services to meet individual client needs through outreach, education, and support, and represents the department and Catholic Charities in San Antonio and Bexar County.
Position Details Work Hours: 8:30 a.m. – 5:00 p.m.
Workdays: Monday – Friday (weekends/evenings as needed)

Note: This is a grant-funded position and salary is dependent on funding availability. The position can be eliminated at any time due to lack of funding to support this position. Responsibilities Conduct comprehensive intake assessments to identify clients’ health needs, barriers to care, and social support requirements; develop individualized service plans to promote access to healthcare and community resources.
Maintain accurate and up-to-date client files, including intake paperwork, assessments and other required documentation.
Provide education and support services to assist clients on how to navigate and access specialized care in their communities.
Provide a culturally and linguistically appropriate orientation, ensuring a respectful and supportive transition into the community with qualified interpretation and translation services.
Develop and implement methods to identify those in need of services.
Research and promote health awareness campaigns to encourage preventive health.
Complete all required professional development in a timely manner.
Be sensitive to the service population’s cultural and socioeconomic characteristics.
Adhere to safety training and protocols daily and take precautions to ensure the safety and well-being of self and others.
Protect the confidentiality of client information, services rendered, donors, investigations, and financial information.
Adhere to the Code of Conduct and the Faith and Moral values.
Take a solution-oriented approach and collaborate as part of a team aligned with the Mission and Vision of the organization.
Other duties as assigned by the Program Supervisor.
Qualifications Minimum Qualifications: High school diploma; at least two years of experience in a health and wellness or social service environment with sensitivity to diverse cultural populations.
Licenses and Credentials: Reliable transportation; Valid driver’s license; Valid vehicle liability insurance; Must be 21 years of age; Clean driving record; Bilingual (preferred languages: Pashto, Dari/Farsi, or Spanish).
Knowledge and Skills: Knowledge of program manuals and operational guidelines; experience with client file documentation and record keeping; computer literacy; ability to work respectfully with people from diverse backgrounds; ability to maintain ethical boundaries and professional relationships; detail-oriented, organized, self-motivated, able to work independently and in a team; strong written and verbal communication skills.
