Qualifications:

Provider must have an active, non-probationary AZ medical license OR have the ability to obtain a full AZ medical licensure At least 5+ years experience working in primary care: internal medicine, geriatrics, or family medicine Current BLS/ACLS certification Family Nurse Practitioners, Adult Gerontology Nurse Practitioners, and Acute Care Nurse Practitioners all welcome to apply
