We are proud of the difference we make in the lives of our patients during their healthcare journeys, and we invite like-minded individuals to explore this dynamic career opportunity.ResponsibilitiesSupports a patient's Plan of Care (POC) by monitoring and recording patient condition, providing support and personal services, and teaching familiesMonitors and assesses patient condition by observing physical and mental condition, intake and output, and exercise. Performs patient assessments and collaborates with the multidisciplinary care team to develop and implement a plan of care.Contribute to the continuity of care through effective communication and partnership with the designated Care Team.Supports patients by providing housekeeping or laundry services, shopping or other household tasks, and preparing and serving meals and snacksProvides personal services as needed by the client, such as bathing, dressing, and groomingHelps family members care for their loved one by teaching appropriate ways to lift, turn, and re-position the patientInterface with family members to field concerns and questions, and educate them on the patient's treatment and aftercare needsRecords patient information via our EMR system and notifies branch leadership of any changes or unusual conditionsEnhances service reputation by accepting ownership for accomplishing new and different requests and exploring opportunities to add value to our accomplishmentsPerforms on-call responsibilities and on-call services to patients/families as assigned with a minimum of 10 hours a week which may include nights, weekend and holidays.Maintains a safe, secure, and healthy environment by following asepsis and universal precaution controls in performing client careProtects the agency by adhering to professional standards, home care policies and procedures, and federal, state, and local requirementsMaintain mandatory continuing education requirements (CEUs) for the agency/licensing board.dMust be committed to 30 hours a week as this position is eligible for all company benefits and all financial incentives.
