Location:  New York City (Multiple Boroughs) Job Type:  Full-Time Compensation:  Competitive, based on experience About the Role We are seeking qualified and compassionate  Care Managers  to support individuals receiving services through NY State programs, including Health Home Plus (HH+), AOT, and HCBS HARP assessments. You will coordinate care, link members to vital resources, and provide services that help clients thrive in community settings. ** Available Care Manager Roles and Requirements** 1. HCBS Assessment (HARP) Responsibilities: Conduct NYS BH HCBS eligibility assessments Ensure clients qualify for home and community-based services Qualifications: Education: Bachelor’s in human services, nursing, therapy, psychology, social work, or a related field OR  NYS licensed RN with bachelor’s degree OR  Any bachelor’s degree + 5 years’ experience with behavioral health populations OR  CASAC certification Experience: 2 years providing direct services or linking individuals to essential community supports Master’s in related field may substitute 1 year of experience Must receive state-mandated UAS and HCBS training Supervision by Master’s-level clinician required 2. AOT (Assisted Outpatient Treatment) Responsibilities: Provide intensive care coordination to individuals under court-ordered behavioral health treatment plans Qualifications: Education: Bachelor’s in approved field*, NYS teacher certificate, or RN with bachelor’s Experience: 4 years working directly with individuals with mental illness or linking them to services Master’s in approved field may substitute for 2 years 3. Health Home Plus (HH+) Responsibilities: Manage complex cases for individuals with serious mental illness (SMI) or substance use disorders Qualifications: Education: Bachelor’s in approved field* OR  RN with bachelor’s OR  CASAC OR  Any degree + 5 years working with behavioral health clients Experience: 2 years in direct service or linking to services for SMI/SUD/DD Master’s may substitute 2 years Supervision: Must be supervised by a licensed healthcare or qualified master’s-level professional Note:  Waivers may be requested for uniquely qualified staff 4. HH+ for HIV/AIDS Members Responsibilities: Coordinate medical and behavioral health services for individuals with HIV/AIDS Qualifications: Education & Experience: Bachelor’s or Master’s in health, social work, education, or mental health + 1 year of experience OR  Associate’s degree, LPN, or RN + 2 years of experience 5. Expanded HH+ Responsibilities: Serve members with more complex mental health needs who require enhanced support Qualifications: Education: Bachelor’s in approved field* OR  NYS teacher’s certificate or RN with bachelor’s Experience: 4 years direct service or linkage to community services for mental health Master’s in approved field may substitute 2 years Bonus Roles: Care Navigators / Peers / Supervisors Care Navigators / CHWs / Peers: HS diploma/GED  OR  CASAC  OR  Peer Certification  OR  CHW credential Must be able to read, write, and follow directions Supervisors: Master’s in health/human services + 1 year supervision + 1 year experience OR  Bachelor’s + 3 years of both supervision and experience Why Join Us?
