Remote Case Manager (Oregon - LCSW / LPC) We are seeking an experienced LCSW / LPC licensed to join a top managed care employer in Oregon as a Remote Case Manager . Pay Details: $75K-$90K ( DOE) + Quarterly Bonus Incentive and Annual Merit Increases

Shift and Location: Monday to Friday, 8:00 AM – 5:00 PM 100% Remote; however, you must reside within 2 hours of an office location: Salem, Springfield, Bend, or Hood River

Daily Responsibilities for Remote Case Manager (LCSW / LPC): Conduct telephonic assessments and develop and implement care plans, ensuring seamless communication among healthcare providers and community partners. Collaborate with providers and care facilities to ensure seamless care navigation and continuity of services for independent caseload.

Required Qualifications for Remote Case Manager (LCSW / LPC): Active LCSW or LPC License in Oregon Reside within 2 hours of an office location: Salem, Springfield, Bend, or Hood River Knowledge of local community services, providers, vendors and facilities available to assist members
