Your responsibilities include welcoming and orienting patients, conducting comprehensive assessments, offering family support, collaborating on individualized care plans, ensuring effective care coordination, and developing crisis management and safety plans.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field.
• Excellent interpersonal and listening abilities.
• Excellent verbal and written communication skills.
• Problem solving, decision making, and critical thinking abilities.
• Ability to stay calm during high-pressure situations.
• Ability to work within an interdisciplinary team.
• Ability to work with young people and their parents/guardians.
• Empathetic, patient, and understanding.
• Must be able to maintain client's confidentiality and adhere to ethical practices.
• Must be able to relate with people from diverse lifestyles and cultures.

• Welcome and create a smooth transition for patients into our care programs.
• Conduct comprehensive interviews to assess patients' mental and emotional needs.
• Offer guidance, support, and information to foster a collaborative approach to patient care.
• Create individualized mental health plans with therapeutic goals and strategies.
• Ensure effective care coordination and comprehensive support for patients.
• Develop crisis management and safety plans for patient safety during challenging situations.
