Screen and assess patients for common mental health and substance use disorders. Track treatment responses and monitor clinical symptoms and treatment side effects systematically. Assist in psychotropic medication management by focusing on adherence and efficacy. Implement brief, evidence-based behavioral interventions tailored to individual patient needs. Engage in regular consultations with psychiatric consultants and integrate treatment recommendations into patient care. Develop self-management plans for patients successfully completing treatment. Qualifications: Valid license as either a Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), or Licensed Marriage and Family Therapist (LMFT) in Washington. Minimum of 3 years of experience treating patients (children and adults) with behavioral health conditions. At least 1 year of experience in a primary care or integrated behavioral health setting. Proficient in assessment and implementation of evidence-based psychosocial treatments. Preferred Skills: Familiarity with the Collaborative Care Model (CoCM). Strong organizational skills and attention to detail. Ability to foster professional relationships with patients and team members. Skilled in telehealth and in-person patient engagement. Adaptability with technology solutions. Spanish language proficiency is a plus.
