Act as a critical part of the care team, providing advanced clinical assessment and developing individualized care plans based on patient needs.
Conduct thorough patient health histories via direct interviews with patients and/or their families.
Utilize specialized knowledge and skills to deliver effective care, identify potential diagnoses, and create targeted interventions.
Maintain accurate, up-to-date patient records and documentation.
Promptly respond to patient inquiries, ensure follow-up on referrals, and provide consistent communication.
Occasionally serve as the designated on-call clinician to address patient emergencies during scheduled work hours.
Collaborate closely with multidisciplinary teams, including primary care providers, specialists, psychiatrists, and community programs, to ensure comprehensive behavioural healthcare for patients of all ages.
What you must have: Minimum of one year (12 consecutive months) of direct patient care experience within the last three years in a public or private healthcare setting.
Demonstrated clinical proficiency with up-to-date competencies.
Must possess a valid photo identification (Driver’s License or Passport).
Current Basic Life Support (BLS) certification through the American Heart Association is required.
Active and valid California Social Worker license (physical copy required
Other Information:
Benefits 6 month contracts with the option to extend
Full-time or Part-time schedules available.
Schedule Flexibility: Options for 8-hour or 10-hour shifts to accommodate work-life balance
Salary/Rate Range:  $90.00 to $100.00/hour
