Provide direct patient care and treatment in a home setting under the supervision of a Registered Nurse. Ensure high quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA).
