JOB SUMMARY: As a Hospice Caregiver , you will provide compassionate personal care to patients in need of support related to daily living activities, under the guidance of a Case Manager (RN). QUALIFICATIONS: Must have a reliable vehicle. At least 12 months of caregiving experience required in settings such as home care, assisted living/memory care, skilled nursing, hospital, or hospice. Hospice experience is preferred. Exceptional customer service skills. Current CPR certification. Valid driver's license and auto insurance. RESPONSIBILITIES (include but are not limited to): Provide personal care (bathing, dressing, grooming, toileting, skin care, etc.). Review and follow patient care plans. Assist with lifting and transferring patients. Support patients with range of motion exercises and proper positioning. Help with dining and nutrition. Complete electronic charting of patient care. In addition, the following attributes are essential: A strong understanding of professional boundaries. Ability to lift up to 50 lbs. Good knowledge of body mechanics. Strong communication skills (written, verbal, listening). Good problem-solving skills. Effective time management skills. Able to function within an electronic medical record system.
