What You'll Do: Deliver skilled physical therapy services to patients in the comfort of their own homes throughout Davidson County, TN . Follow individualized plans of care developed by a Physical Therapist. Promote mobility, independence, and wellness. Collaborate with a multidisciplinary care team. Requirements: Qualifications: Graduate of an accredited PTA program Valid Tennessee PTA license Current CPR certification Valid driver’s license, auto insurance, and reliable transportation Minimum 2 years experience in a healthcare facility or similar setting, home health experience preferred Strong written and verbal communication skills Working Conditions: The majority of work is performed in providing patient care services in the patient’s home.
