You will conduct evaluations, develop care plans, and educate patients and their families to ensure safe and effective therapy at home. Key Responsibilities: Manage a caseload of home health patients needing physical therapy. Conduct comprehensive assessments and develop individualized care plans in collaboration with patients, families, and physicians. Regularly reassess patient conditions and adjust care plans as necessary. Educate patients and families on therapy techniques for optimal recovery. Document patient progress and communicate effectively with the healthcare team.
