Schedule: Full Time Shift: Monday-Friday 8:00 AM - 5:00 PM (Rotating weekends) Location: This position conducts patient home health visits in the Hardee County area. Your Role: As a Licensed Practical Nurse, you will administer nursing care according to physician’s orders and under the guidance of a Registered Nurse. You will provide individualized nursing care, educate patients and their families, and ensure continuity of care. Your role involves performing technical nursing skills proficiently and supporting a collaborative approach to patient care. You will also execute physician's orders and communicate any necessary updates to the RN or Case Manager while completing documentation in compliance with agency policies. The Value You Bring to the Team: Adhere to all department policies, procedures, practices, and standards of care. Provide direct patient care efficiently while collaborating with the RN. Perform treatments and administer medications as per training and scope of practice. Conduct relevant data collection specific to patient needs, contributing to their plan of care. Report observations to the attending physician and RN, implementing necessary changes as directed. Receive, verify, and implement physician orders, ensuring RN co-signing where required. Communicate effectively with all team members, patients, and families. Maintain clinical records in line with established standards and timelines. Qualifications for Success: High School Diploma or equivalent Graduation from an Accredited School of Licensed Practical Nursing Current state LPN licensure Active American Heart Association BLS certification Valid state Driver's License with current car insurance IV Certification This facility is an equal opportunity employer and complies with all relevant anti-discrimination laws.
