SUMMARY OF POSITION
The purpose of the Health Information Technician is to represent the Medical Records Department by performing exceptional quality customer service and overall support of the medical records file coordination in compliance with federal, state, local requirements and accreditation, as well as according to hospital policy. Hospital Operator for incoming and outgoing calls.
RESPONSIBILITIES & EXPECTATIONS
Assist both internal and external customers with exceptional customer service skills.
Coordinate loose documentation:
1. Verify loose documentation is part of the medical record.
2. Return documentation that does not belong in the medical record to the appropriate author, department or disposal method according to hospital policy.
3. File in proper patient's chart.
4. File in proper location of the chart.
5. Scan documentation to patient encounter by date of service
Procurement of records:
1. Process records of expired patients.
2. Purge records to offsite storage according to purge schedule.
3. Prepare records for destruction according to destruction schedule.
Copy transfer charts.
Process fax requests for release of information.
Perform data entry for accurate record filing and retrieval to offsite storage system.
Provide notary services.
Auditing charts for documentation and scanning.
Coordinate record out-guide system:
1. Locate.
2. File.
3. Retrieve.
4. Out-guide.
Produce labels for permanent folders.
Process transcription services STAT documentation for timely completion.
Prepare chart list and retrieve charts for internal and external customers.
Process the following Release of Information (ROI) requests in a timely manner according to HIPAA regulations.
Determine records to be released by reviewing requestor information.
Process through ROI vendor.
2. STAT requests.
3. Denial requests.
4. Audit requests.
Determine records to be released by reviewing requestor information.
Escalate all internal/external customer issues to appropriate leadership that may delay the timely release of records.
Perform other duties as assigned.
Process ROI for Dunlap Family Practice (Orrville, Applecreek, and Dalton)
Process ROI for Affinity Family Practice for three providers.
Process ROI for AOH Carrollton Health Center
Process ROI for AOH Bolivar Health Center
Process ROI for Pain Clinic through Athena and Cerner.
Hospital Operator for incoming/outgoing calls.
JOB REQUIREMENTS
High School Diploma
Work independently and in a team environment
Strong auditory, verbal and written communication skills
Organize, prioritize and multi-tasks responsibilities
Strong computer skills
Certified Notary
Medical Terminology
Subject to background check and fingerprinting

Aultman Orrville Hospital Operator and ROI Monday-Friday 8:00 to 4:30
Subject to frequent interruptions and changes in priority of duties throughout the day
