Responsibilities As a Nurse Practitioner, you will perform annual risk assessments in residential settings or via telehealth, following the guidelines set by our Collaborative Physician. Primary Duties Include: Conducting thorough in-home health risk assessments to identify both active and chronic conditions, along with assessing physical, mental, and social needs. Collecting patient history, performing examinations, and establishing diagnoses. Documenting patient visits in compliance with NCQA standards. Measuring patient vital signs when necessary. Coordinating case management referrals and communication with primary care providers as needed. Engaging with patients, caregivers, agency nurses, and other providers to ensure accurate diagnosis. Adhering to OSHA Universal Precautions during all clinical duties. Maintaining patient confidentiality consistently. Attending scheduled meetings and trainings, and participating in committees as required. Engaging in professional development activities to uphold licenses and affiliations. Current CPR Certification. Board eligible or certified in family medicine. Strong EHR skills. Preferred Skills and Experience: Experience or training in geriatrics. Proven ability to work effectively within a team and build strong relationships with patients and staff. Conditions of the Role: Must be adaptable to varying weather conditions and different home environments. Ability to perform a full range of physical activities, including handling and lifting patients while maintaining manual dexterity and coordination. Regularly lifting/carrying items up to 50 pounds. Ability to travel daily up to 150 miles in urban or rural settings; driving if necessary. Compensation: Minimum Pay Rate: $128,000.00/Yr. Maximum Pay Rate: $134,000.00/Yr. Notice: HarmonyCares and HarmonyCares Hospice are distinct entities and do not operate under the name Harmony Hospice Care. Our Hospice services are offered in MI, VA, WI, TX, IN, and IL, but do not extend to OH.
