OPEN FOR IMMEDIATE PLACEMENT At Mountainlands Community Health Center we are health professionals assuring the provision of high quality, primary healthcare for everyone in our community. We serve diverse populations, including underserved and vulnerable groups, and are committed to promoting whole-person wellness through integrated primary medical and behavioral health services. We are seeking a compassionate and dedicated Mental Health Therapist to join our team. The Therapist will provide individual, couples, and family counseling services to clients across the lifespan. This role includes assessment, diagnosis, treatment planning, treatment, crisis intervention, and collaboration with medical and support staff to ensure comprehensive, coordinated care. Key Responsibilities: Conduct biopsychosocial assessments and develop individualized treatment plans Provide individual, couples, and family therapy Diagnose mental health conditions in accordance with DSM-5-TR criteria Maintain timely and accurate clinical documentation in the electronic medical record (EMR) Provide crisis intervention and risk assessment as needed Collaborate with primary care providers and other team members in an integrated care model Participate in case conferences, team meetings, supervision, and peer/chart reviews Support care coordination and referrals to community resources Comply with all ethical, legal, and regulatory requirements Qualifications: Master's degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy Required Current licensure (e.g., LCSW, LMHC, LMFT) or eligibility and active pursuit of licensure, if applicable Required Proficient English and Spanish communication skills Required Comfortable working with a range of individuals from different ages (children, teenagers, adults), diagnostic conditions, and other diverse populations Knowledge of trauma-informed care and evidence-based treatment modalities Experience working with individuals experiencing substance use disorders Strong documentation and organization skills Skills and Competencies: Cultural humility and commitment to equity Strong interpersonal and communication skills Ability to work independently and as part of a multidisciplinary team Crisis management and de-escalation skills Proficiency with EMR systems Benefits: Competitive salary Flexible schedule Affordable health, dental, and vision insurance (company pays 80%) Generous paid time off and 11 paid holidays Retirement plan with employer contribution and match Continuing education support MCHC is a Patient Centered Medical Home.
