This role involves managing 12-15 patients daily, emphasizing preventive and outpatient senior care within a multidisciplinary team. The position requires active NP or PA licensure, DEA certification, and experience with geriatrics and value-based care models. CenterWell is looking for a dedicated Advanced Practice Provider who is Board Certified/Board Eligible for an exciting primary care role. A Day with CenterWell: See 12-15 patients per day, allowing for personalized patient interactions Play a crucial role in a value-based care model, boosting patient's quality of life Handle a diverse caseload from preventive care to chronic conditions. Dedicate 100% of your time to direct patient care, including patient-facing time and associated administrative tasks Active, unrestricted NP/PA license in the state Current NP or PA certification Current DEA license or the ability to obtain one Passion for enhancing consumer experiences within an organization This role is patient-facing and part of Humana/Senior Bridge's TB screening program. TB screening will be required.
