They coordinate and evaluate individualized care plans for patients recovering at home, ensuring effective communication with families and caregivers. This role requires a NYS RN license, nursing degree, and experience in acute medical-surgical nursing, with preference for Certified Home Health Agency experience. Fee for Service rates: Initial & ROC Weekday visit rate: $100 Inital & ROC Weekend/Holiday visit rate: $120 Revisit daily rate: $85 Orientation daily rate: $250 In-Service hourly rate: $55 Weekday and weekend shifts available.
