Responsibilities:Responsible for providing direct care to Members under the immediate direction of the Charge Nurse in accordance with the Member care plan, established nursing policies and proceduresParticipates in the admission processes of Members to include, but not limited to, greeting and escorting Members to their room, introducing Carroll Center staff, assisting Member in storing clothing and assisting Member in becoming acclimated to their room and other areas of the Carroll Center. Assists in discharge process as necessaryResponsible for ensuring compliance will all current rules, regulations and guidelines that govern longer term care facilitiesActively participates in emergency preparedness programs and responds accordinglyResponsible for ensuring that work assignments are performed within acceptable nursing standardsMust report all complaints and grievances made by Members to the appropriate personResponsible for assuring that appropriate, informative, and descriptive information is communicated between nursing personnel on different shifts to include, but not limited to, reporting all accident/incidents involving Members, reporting unauthorized discharges in accordance with established departmental procedures, and reporting all discrepancies noted concerning established Member careMust request daily assignments form Charge Nurse upon reporting for shiftAssist Members with daily hygiene to include, but not limited to, mouth care, bathing, dressing/undressing, grooming, bowel and bladder functions and assisting with meals selections and consumptions as necessaryRecord usage of ancillary and medical equipment for Members to ensure proper billing as per current systemAssist with lifting, repositioning, moving, and transporting Members into and out of beds, chairs, bathtubs, wheelchairs, lifts and other itemsAssist Members to walk with or without self-help devices as instructedPerform services as assigned that may include, but is not limited to, obtaining sputum, urine and other specimens, taking and recording vital signs and weights, bathing, observe and report early signs of skin breakdown, indwelling catheter care, lifting and repositioning, and palliative careAssist Members with meal services to include, but not limited to, in room food service, assisting with feeding as needed, preparing Members for meals and providing after meal care, and reporting and recording changes in Member eating habits and/or fluid intakeEnsure that Member rooms are clean, sanitary, and safe. Make beds, change bed linens and place extra covers on beds as requestedAdhere to all aspects of the Carrol Center written Member Rights programContinuously follow company procedures to include, but not limited to, risks and prevention of infectious disease, proper hand washing techniques, proper and safe use of work tools, equipment and other procedures, proper use of personal protective equipment, hazard communications, safety, universal precautions and bloodborne pathogens programsAssist other employees with lifting and/or carrying unusually large or heavy objects in a manner that complies with company safety proceduresCommunicates as needed with Members to resolve problems or complaints in a timely and effective manner that facilitates a high level of Member careResponsible for using Member care equipment and supplies in a safe manner in accordance with established departmental policies and proceduresCreating and maintaining an atmosphere of warmth, personal interest and a calm environment throughout the unit and shiftActively participates in mandatory meetings on the units and other training sessions as assigned. Attend and participate in continuing education programs designed to keep staff abreast of changes as well as to maintain licenses in a current and active status.Functions as a preceptor to new staff members as assignedMaintains confidentiality of members' PHI (Protected Health Information) and medical records information as wellOther various duties as assignedQualifications:Must have at least an active certificate as a Certified Nursing Assistant in the State of Florida. High School diploma or equivalent preferred.Six (6) months proven experience as a Certified Nursing Assistant in skilled nursing, acute care or assisted living preferred, but not requiredSkill in understanding of computer software as it relates to the job.
