About the Role

As a Certified Home Health Aide (CHHA), you will provide essential care and support to patients in the comfort of their homes. Your work will help promote independence, comfort, and dignity, ensuring the highest standard of care for your patients and their families.

Responsibilities Include:
  • Measuring and documenting vital signs during every visit.
  • Assisting patients with personal hygiene, such as bathing, grooming, and oral care.
  • Supporting patients with mobility, including transfers, ambulation, and prescribed exercises.
  • Preparing nutritious meals, including therapeutic diets, and assisting with feeding.
  • Helping with medication reminders (no administration of medications).
  • Performing light housekeeping tasks such as changing linens and maintaining a clean environment.
  • Observing and reporting changes in the patient's condition to the supervising nurse or therapist.
  • Following care plans and assisting with specific patient needs under professional guidance.
What We're Looking For:
  • Certification as a Home Health Aide by the Department of Health Services.
  • Proficiency in English, both verbal and written.
  • Demonstrated competency in providing care and performing assigned tasks.
  • Strong interpersonal skills to build trust and rapport with clients and team members.
  • Drivers License and reliable transportation
