Lead Behavioral Health Care Manager

The Lead Behavioral Health Care Manager will oversee a team of BHCMs to provide high quality and effective care management to Titanium Healthcare members. The BHCM Lead will be responsible for the care management process: outreach and engagement to members, engaging members in skilled therapeutic interactions to promote health behaviors, coordination of care, resource linkages, working with other professionals and organizations in the community to ensure quality of care for members, seamless transitions of care, and facilitating proper care for members.

This position is hybrid. You will work from home and service members in-person. A flexible schedule is required with core hours aligned to business needs and health plan engagement. Requires reliable transportation, valid driver's license, insurance, and ability to travel within assigned Inland Empire regions. Standard business hours are Monday through Friday from 8:30 am to 5:00 pm.

What You'll Do

  • Serve as the lead supervisor for all Behavioral Health Care Managers, offering direct oversight, support, and mentorship
  • Coordinate and align operational goals across the BHCM team to ensure consistency and quality in care management delivery for IEHP members
  • Ensure BHCM's are meeting performance metrics, quality assurance standards, and compliance requirements as defined by Titanium Healthcare and IEHP
  • Lead weekly huddles with BHCM's to assess team performance, address barriers, and implement improvement strategies
  • Develop and deliver training, tools, and best practices to support staff development and program excellence
  • Monitor complex behavioral health cases and escalate cases to LCSW for clinical oversight or case consultation as needed
  • Collaborate with cross-functional teams including physical health care managers, social workers, and community health workers to ensure holistic member support
  • Identify system gaps, propose process improvements, and drive initiatives to enhance service delivery across the behavioral health team
  • Perform other duties as assigned or required

Who You Are

  • Fluent in English (written and verbal)
  • Demonstrate leadership and team management abilities, especially in tiered or matrixed structures
  • Strong organizational, analytical, and decision-making skills
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively across departments and with external partners
  • Proficient in electronic health records (EHR) systems, case management documentation platforms, and other computer system software including Microsoft Office
  • Knowledge of trauma-informed care, motivational interviewing, and culturally competent practices

What You'll Need

  • Master's degree in social work, psychology, counseling, or related behavioral health field required
  • Minimum 1 year of experience in behavioral health care management leading a team
  • Current and valid Driver's License
  • Proof of auto insurance
  • Distraction-free home workspace with a secure internet connection

Nice To Haves

  • Experience using eClinicalWorks (ECW) experience a plus
  • Experience working with Medi-Cal populations and knowledge of ECM/CalAIM preferred
  • Prior experience working with or alongside IEHP or similar managed care organizations is highly desirable
