Job responsibilities:

Provide patient and family education related to the plan of care and discharge needs. Document assessments, interventions, and patient progress in the medical record. Work 36 hours per week with 13-hour shifts. Assess patient status and respond to changes using established clinical protocols. Support safe transitions of care, including handoffs and care plan updates.
