Position Summary:

VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Santa Rosa, CA. Key Responsibilities:

• Provide home health physical therapy services to adult patients
• Conduct up to 6 patient visits per day meeting 30 productivity points standard
• Maintain current BLS certification and state PT licensure
• Utilize Epic and Oasis charting systems for documentation and patient assessments
• Provide physical therapy services for surgical patients including back, neck, total joint and neurological cases
• Develop and implement home exercise programs for patients
• Provide patient and family education on treatment plans and body mechanics
• Work with patients with various conditions including post-operative cases, spinal conditions, total joint replacements, neurological disorders, and traumatic injuries
• Complete required pre-start and annual training modules within specified timeframes
• Self-report time spent on non-billable training modules
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty setting
• Maintain reliable transportation for patient visits with occasional travel radius of 50+ miles
• Work one weekend shift per contract as required
• Float between facilities within the state as assigned
• Complete required compliance documentation and certifications
• Utilize facility-provided communication devices when required for patient care coordination

Job Requirements:

• Active, unencumbered license in CA
• Minimum of 1-7 years of relevant experience, depending on specialty
• Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
• Experience with EMR systems (Epic, Cerner, as applicable)

Other Required Documentation:

• Current Resume listing detailed work history (MM/YYYY format)
• Two current clinical references
• State-required professional license
• Physicial exam (completed within the past 12 months)
• TB skin test or Quantiferon Gold (within the past 12 months)
• Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
• Current Tdap and Flu vaccinations
• Must pass background check and drug screening
