Required Qualifications
Physical Therapist, Home Health Department: Physical Therapy– Home Health
- # of patients treated per day: 5-7
- Experience or 'Must have' skills: Must have PT related skills & previous home health experience
- # Years Experience Required: 1+year
- Previous Travel experience Required?: No
- Common diagnosis / Types of patients: Home health clients - surgeries, falls, etc.
- Acute Inpatient, Acute Rehab, Skilled, Outpatient?: Home Health and Hospice patients
- What areas are they required to cover?: 60mi radius around Valley City, ND
- EMR: HomeCare/HomeBase
- Is there a PT Manager/Supervisor on each shift?: No
- Do you staff Rehab Aides: PTAs?: This is a possibility
- Shift Details: Monday - Friday 8a-5p
- Do you allow self-scheduling? n/a
- Do you allow Block scheduling?: n/a
- Weekend Requirements: No

- Scrub Color/Dress Code: CSH color scrub or Business Casual
