Job Summary
Provide psychosocial assessment, care planning, counseling, and resource coordination for home health and hospice patients across the lifespan. Participate in weekly care conferences and the interdisciplinary team (IDT), ensure regulatory compliance, and support bereavement services. This salaried role also provides limited on-call coverage for the hospital every other weekend (4 hours/day) and occasional coverage for acute care or behavioral health when needed. Reports to the Chief Nursing Officer / Home Care & Hospice Manager.
Key Responsibilities
- Complete timely psychosocial assessments; identify social, emotional, financial, and environmental needs; contribute to the plan of care.
- Educate patients/families on advance directives; verify, document, and communicate patient wishes in the medical record.
- Provide counseling, crisis intervention, and goals-of-care support; assist the team in meeting the needs of patients at end of life.
- Coordinate community resources (food, medical supplies, financial aid, long-term care placement) and address safety concerns in the home.
- Participate in weekly care conferences/IDT, document visits and education, and maintain accurate social service records.
- Support bereavement programming and volunteer training as needed; ensure hospice regulatory requirements are met.
- Identify and report potential quality, risk, or utilization issues; engage in program evaluation and improvement.
- Provide limited hospital on-call support (every other weekend, 4 hours/day) and occasional coverage for acute/behavioral health social work.

Job Qualifications
Required
- Arkansas LCSW or LMSW (under LCSW supervision).
- Basic Life Support (BLS) certification.
- Excellent verbal and written communication skills in English; ability to assess psychosocial needs and respect patient rights and autonomy; adherence to the Arkansas Social Work Licensing Board Code of Ethics.
Preferred
- Previous healthcare social work experience; familiarity with Meditech or similar electronic health record.
Compensation & Schedule
- Status: Full-time
- Schedule: Primarily weekdays; every-other-weekend on-call (4 hours/day) for hospital coverage; occasional acute/behavioral health coverage as needed
