Pay Rate: $80/hr

Establishes and implements care plans by assessing residents' social, emotional, and family needs; sets goals with residents and families to promote independence and well-being. Provides medically related social services to help residents attain or maintain the highest practicable physical, mental, and psychosocial health. Offers emotional support, counseling, and assistance with coping related to disabilities, health concerns, grief, and loss. Assists residents with healthcare decisions, discharge planning, and coordination with outside agencies for services, referrals, and resources. Coordinates ancillary services such as optometry, podiatry, dentistry, and psychiatric appointments. Helps residents with personal needs such as shopping, belongings inventory, and resolving issues of lost/stolen items. Secures prosthetics and assistive devices to support resident care and quality of life. Maintains documentation of resident social service status and participates in interdisciplinary team meetings. Builds strong working relationships with facility departments and community agencies to ensure comprehensive support. Provides in-service training to staff, promoting understanding of resident social service needs and effective interventions.
