Summary Purpose for the Position:
Responsible for building trust and enhancing the communication among residents, responding to service needs, and developing the leadership capabilities of residents in targeted properties.
Requirements (Education, Experience, Certification, Knowledge, Skill):
Requires at least three to four years of education or training and/or experience in the fields of human services or a related field.
Knowledge of the human service community and resident service issues is also required.
Demonstrated ability to work independently, cooperatively and with people of diverse background.
Position Responsibilities:
Position is responsible for maintain resident records using PathStone’s IFDP system and completing activity reports.
Excellent communication, networking and community organizing skills.
Must acquire knowledge concerning ethnic composition and primary language of the residents in assigned properties. Spanish language may be required for some properties.
Flexibility and willingness to work flexible hours to meet the needs of residents
Evaluate service needs of residents in targeted sites.
Develop service system, act in case management capacity and monitor resident progress
Identify community and neighborhood resources to effectively refer resident to programs and services outside the PathStone service network
Established linkages and serves on coalition to coordinate with other relevant programs and agencies.
Attendance at agency staff meetings and other program and agency meetings.
Collaborates with staff and management6 team to accomplish program goals
Conducts individual interviews and resident meetings
Evaluates PathStone’s current method for communicating/interacting with residents and develops comprehensive strategy to enhance current methods
Develops strategies for enhancing trust and communication among residents
Develops strategies for enhancing residents links with general community
Evaluate current system for responding to resident inquires/complaints and make necessary improvements
Assist residents in defining personal goals and developing a plan of acting using PathStone IFDP model
Assist resident in defining individual/group goals as they relate to quality of life where they live
Encourage and support development of resident leadership structures.
Order supplies, monitor petty cash
Develop methods and procedures for responding to resident needs in meeting self-sufficiency goals, fulfilling quality of life goals with property where they reside and in developing resident leadership goals.
Establish regular schedule of resident recreation and resident education and development activities at assigned properties
Develop effective resident services plan in assigned properties.
Develop and maintain case management process
Enter all appropriate resident information gathered through use of IFDP in the MIS system
Provide effective outreach and information to residents on PathStone and community services
Develop and maintain strong community networks
Maintain accurate record-keeping system for individual and group resident activities
Traveling between assigned sites required
Will conduct resident outreach and interviews in office, community room and in homes
Evaluate trainings needs of residents
Coordinate or provide training based on resident needs
Be fully knowledgeable of PathStone services and programs
Be fully knowledgeable of community resources
Acts as an advocate and facilitator to assure resident access to public services and benefits for which they are eligible
Working Conditions/Environment:
Perform home visits which involve a high degree of exposure to injury, hazardous materials, and/ or infectious environments.
Traveling for meetings and training’s is required.
Transportation Requirement:
Position requires automobile, driver’s license and insurance.
