What You’ll Do:

Visit clients in their homes to assess mobility, balance, and safety

Develop and implement individualized care plans and evidence-based interventions

Provide education to clients and families to support independence

Collaborate with a multidisciplinary care team

Document care electronically using a state-of-the-art tablet

What You’ll Need:

Active Delaware PT license

Graduation from an accredited PT program

Ability to work independently, with strong communication and time management skills
