About Us Wise Medical Staffing provides high-quality, compassionate non-medical home care services, helping clients remain safe, comfortable, and independent in their own homes. We are seeking a PRN Licensed Practical Nurse (LPN) to oversee and support the delivery of care by our home care team, ensuring services meet client needs and comply with company standards.

Job Summary The LPN in this role will focus on client assessments, care plan oversight, and caregiver support—not skilled nursing procedures. You will work closely with caregivers, clients, and families to promote quality care and a positive home environment.

Qualifications Graduate of a state-approved school of practical (vocational) nursing

Current LPN license in the state of Indiana

Minimum of one (1) year of experience in nursing or home care preferred

CPR Certification (Required)

Strong communication and organizational skills

Commitment to providing compassionate, person-centered care

Responsibilities Conduct initial and periodic client assessments for non-medical home care services

Develop and update care plans in collaboration with clients and families

Provide guidance and training to caregivers regarding client care needs

Monitor client satisfaction and report changes in condition to appropriate parties

Maintain accurate documentation of visits, assessments, and care plan updates

Participate in staff meetings and ongoing training

Serve as a liaison between clients, families, caregivers, and agency management

Pay & Schedule

Pay: $26.00 – $30.00 per hour (based on experience)

Job Type: PRN (as-needed)
