Physical Therapist at Heartland Home Health summary: The Physical Therapist delivers skilled physical therapy services to patients recovering from injuries, illnesses, or disabilities, working closely with physicians and clinical teams to develop and adjust treatment plans. They implement conditioning and rehabilitation programs aimed at maximizing patients' physical function. This role requires a Master's degree in Physical Therapy, state licensure, and CPR certification, with experience preferred in hospital or home health settings. Overview: We are looking for a Physical Therapist to join our team. The Licensed Physical Therapist assumes responsibility and accountability for the delivery of appropriate physical therapy services within a framework of interdisciplinary collaboration and consultation. Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses, and functional disabilities to attain the highest level of physical function. Confer with physician and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment. Develop/implement a conditioning/rehabilitation program consistent with the physicians' Plan of Treatment and the overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.
