The In-Home Outpatient Manager is a licensed Physical Therapist and seasoned leader responsible for supervising, supporting, and evaluating our In-Home Outpatient rehabilitation team, which includes Physical, Occupational, and Speech Therapists. This pivotal role ensures clinical excellence, manages workflow, drives referral growth, and builds strong relationships within Assisted Living Facilities (ALFs) and the broader community. In addition to delivering direct patient care as needed, this leader ensures high-quality service delivery, manages team operations, and collaborates closely with Business Development and Clinical leadership to grow the IHOP service line and drive cross-referrals across RVNAhealth's full continuum of care. Key Responsibilities Clinical & Operational Management Oversee and coordinate IHOP rehabilitation services and staff scheduling. Review referrals and assign therapists appropriately. Provide clinical visits when needed and assist with complex cases. Support intake and maintain accurate territory staffing charts. Ensure compliance with documentation, billing, HIPAA, and Medicare regulations. Analyze and act on metrics, KPIs, and quality benchmarks. Conduct chart audits, staff performance evaluations, and mentor new hires. Leadership & Team Development Supervise and develop IHOP therapy team including onboarding, training, and 90-day/annual reviews. Facilitate regular team meetings, case conferences, and in-services. Provide hands-on support, guidance, and clinical oversight. Manage PTO requests, weekend rotations, and maintain adequate staff coverage. Business Development & Community Outreach Collaborate with Business Development and ALF account managers to increase referrals and service awareness. Represent RVNAhealth at ALF clinical huddles and community events. Ensure smooth transitions of care and clear communication with referral sources. Generate and analyze monthly IHOP service line reports and contribute to annual budgeting and goal setting. Qualifications Bachelor's or advanced degree from an accredited Physical Therapy program. Current CT Physical Therapy license in good standing. Minimum 5 years of clinical experience , with at least 2 years in In-Home Outpatient care . Strong understanding of Part B billing practices , home-based therapy, and compliance standards. Proven leadership ability with experience supervising clinical staff. Excellent communication, organizational, and relationship-building skills. Proficient in clinical documentation systems (e.g., WebPT, HCHB) and outcome reporting. Why RVNAhealth?
