Key Responsibilities

• Provide physical therapy treatments under the direction of the Physical Therapist
• Follow individualized treatment plans and physician orders
• Document daily treatment notes and weekly progress notes in accordance with PT Board guidelines
• Communicate patient progress with the therapy and interdisciplinary care team
• Participate in care plan meetings and rehabilitation conferences as needed
• Educate patients, families, and nursing staff on therapy programs and discharge planning
• Assist with training restorative nursing aides when appropriate
• Maintain therapy equipment and ensure the treatment area is clean and organized

Qualifications

• Active California Physical Therapist Assistant (PTA) license
• License must be in good standing
• Experience in skilled nursing, post-acute, or rehabilitation settings preferred
• Familiarity with Casamba, PointClickCare, or similar EMR systems is a plus
• Strong communication and teamwork skills
• Passion for helping patients regain independence and mobility
